Special Part of the Sole Agreement
The Special Part of the Sole Agreement (hereinafter referred to as the ‘Special Part’) describes the special conditions of the Agreement applicable to a particular customer, i.e. contact details of the Parties, data source (register and/or information system), if services are provided, the name of the service, data (services) provided, method of data (services) provision, legal basis for obtaining data (services), payment conditions applicable in the individual case, etc.
The Special Part may provide for exceptions to the conditions of the General Part of the Sole Agreement (hereinafter referred to as the ‘General Part’) or for amendments to the conditions of the General Part applicable to a particular customer if the Parties agree on such amendments.

General Part of the Sole Agreement
The General Part defines the concepts used in the Sole Agreement on the Provision of Data and Services, the rights and obligations of the Parties to the Agreement, establishes general conditions for payment and Data User administration, procedures for amending and supplementing the Sole Agreement on the Provision of Data and Services, requirements for processing personal data, etc.
The conditions of the General Part shall apply to the provision of data and services for all registers and information systems in the same way.
The conditions of the General Part shall not be amended, except in cases where exceptions or amendments to the to the conditions of the General Part applicable to a particular customer are provided for in the Special Part.

- Confidentiality Commitment
This is the template for the customer to draw up the Confidentiality Commitments and submit them for the signature of their employees for accessing the register and/or the information system.
- Application for the Administration of the List of Data Users
This is a template to be completed by the customer for the administration of Data Users (adding a new user, deleting the existing user, clarifying existing data).
The provision of data under the newly signed agreement shall be activated only when the signed agreement is accompanied by a duly completed and signed application for the administration of Data Users. After signing a new agreement, please submit a completed and signed application for the administration of the list of Data Users together with it.
